Food Handling Permit - If food, non-alcoholic and/or alcoholic beverages are to be distributed on a temporary or permanent basis, you will need approval from the Health Department.
Federal and State Forms Needed to Get Your Business Started
The first thing to do is call the Small Business Hotline, 1-800-252-2923 to receive a Business Start-up Kit. Items included in the kit are essential information and forms to start a Business in Illinois. A few key areas are highlighted below.
- Federal Employer Identification Number, Form SS-4, can be obtained from the IRS, located at the Federal Building, 250 W. Cherry, Carbondale, IL 62901. When your SS-4 has been processed, you will receive your Employer Identification Number, your Federal Tax Deposit Slips - FTD 501 and FTD 508- and relevant information about withholding taxes. For further information, call toll free, 1-800-424-FORM (3676).
- Illinois Certificate of Registration registers you as a Business in the State of Illinois and requires you to collect a sales tax on retail and certain other types of activities. You may be required to post bond for the first 3 months’ estimated sales tax revenue. An application can be obtained from the Illinois Department of Revenue, located at 1107 West DeYoung Street, Marion, IL 62959, 618-993-8183.
- Assumed Name Certificate - Businesses in Jackson County are required to file an assumed name certificate with the County Clerk’s Office and have the name published in local newspapers. Forms are available from the County Clerk’s Office, located at the Jackson County Courthouse, Murphysboro, IL 62966, 618-687-7360.
- If you plan to form a corporation through which you would conduct Business, a Business Corporation Application, Form BCA-47, must be filed by all domestic corporations with the Illinois Secretary of State’s Office. An annual report is then required by March 1 for all corporations after the initial year. Mail to: Illinois Secretary of State, Centennial Building 3rd floor - Corporations, Springfield, IL 62756, 217-782-7880.
- Report to Determine Liability for Unemployment Insurance, Form UC1 - Every Illinois employer is required to file this report with the Illinois Department of Employment Security, Division of Unemployment Insurance, 223 S. 13th St., Murphysboro, IL 62966, 618-687-2341.

Building and Development
The Development Services Department of the City of Carbondale is prepared to guide you through the development process. All City offices can be reached at 618-549-5302 or online at www.explorecarbondale.com
The following is a brief overview of development issues:
- Determine if the type of Business you plan is permitted at the location selected. This can be accomplished by contacting the City of Carbondale Planning Services Division on the second floor of City Hall at 200 S. Illinois Ave. If the site is zoned for your use, obtain a zoning certificate. If a rezoning is needed, allow eight weeks lead time because of the requirements of State law.
- If your project involves expanding or building a new structure (other than one house or duplex on a lot) or building a parking lot for more than 4 cars, a site plan will be required and must be approved by the City. Site plan review usually occurs within one week after the site plan is submitted. Site plan requirements and applications are available from Planning Services.
- If your project includes new construction, an addition, or remodeling to a structure, a building permit is required from the City. Required electrical, plumbing, and mechanical permits are also issued by the Building and Neighborhood Services Division.
- If your project includes signage, a sign permit is also required. Before ordering a sign, have your sign-maker check with the Building and Neighborhood Services Division to be sure the sign meets code standards.
- If you do not have a specific location selected for your Business, time can be saved by contacting a local realtor and explaining your needs. A list of realtors can be obtained from the Carbondale Chamber of Commerce at 618-549-2146, or found in the yellow pages of the telephone directory.
